Feel confident self-managing diabetes with our new support program


It's easy to feel overwhelmed managing a chronic condition like diabetes. But you don’t have to figure it out alone—we can help (in fact, that’s what our team is here for!) The Vera Whole Health Diabetes Education Care & Program provides clinical support, tailored to your specific needs.

You’ll get to work with a friendly and knowledgeable Certified Diabetes Care & Education Specialist (CDCES)—who is also a Registered Dietitian (RD)—virtually 1:1 or in group sessions for you-centered education and support through two services, depending on the kind of support you need.

Best of all? The program is available to Vera patients at no cost.

Vera Whole Health is a separate, HIPAA-compliant, primary care provider and will keep your health information confidential. Your employer will not have access to your health information. Vera services are available to SAPPT members, their spouses/domestic partners and dependents (ages 2 and older when visiting Fort Dent Tukwila Care Center; age 3 and older when visiting all other care centers) enrolled in the Aetna PPO plan.

The Diabetes Care & Education Program is open to members and their families with a referral from a Vera provider or CDCES. Members/patients must reside in the same state where the CDCES, RD is licensed; WA, AK, OR, ID, AZ, CA, MO, KS
